<p>This seminar will review these changes starting with a review of the revised RAI/MDS manual and assessment requirements. Additionally, our presenter will assist our attendees to:</p>
<ul>
<li>Understand the types and scheduling of assessments for OBRA requirements including payment drivers and care planning.</li>
<li>Review MDS Resident Interview items and requirements for unplanned discharges.</li>
<li>Know the new requirements for facility documentation.</li>
<li>Be aware of the changes to Care Area Assessment documentation requirements.</li>
<li>Have an understanding of MDS items that influence QI/QM. Review of the QM reports and the four new surveyor QMs.</li>
<li>Be shown examples related to early and late COT and EOT PPS MDS and its impact on billing.</li>

<p>Reducing acute care transfers and hospital readmission rates is being evaluated across the country as a quality measure and improvement initiative that should be focused on by all long-term care facilities. The Massachusetts Senior Care Foundation addressed the issues this way: “Although [hospitalization is] often appropriate, studies have suggested that some residents could be cared for more cost effectively and with better clinical outcomes by remaining with trusted caregivers in a familiar nursing facility setting.”</p>
<p>With backing from CMS, the special project known as INTERACT was started in 2008 in three Georgia nursing facilities which demonstrated a significant reduction in unnecessary hospital transfers, using a tool kit developed by a team led by Dr. Joseph Ouslander, a professor and Associate Dean for Geriatric Programs at the Charles E. Schmidt College of Medicine at Florida Atlantic University.</p>
<p>INTERACT II is a refinement of the original work and is a series of educational and clinical tools which aim to helpnursing facility staffs safely manage residents’ acute conditions rather than reflexively transfer them to the hospital.</p>
<p>Join Dr. Ouslander and his colleague, Laurie Herndon, G.N.P., as they present this Quality Improvement program and review the tools for implementation.</p>
